Work Permit Information
UNDER 18 YEARS OLD
- All youth (14 – 17 years of age) must download and complete the youth application found at http://www.mass.gov/dos/youth/youth_application.pdf
- The application must specify what type of schooling the youth is receiving (GED Prep, Day School, Night School, or Program Attending).
- The application must also specify the number of hours, either daily or weekly, that the youth will be working.
- All youth must have a Parent/Guardian Signature on the application before submitting to the School Department.
- When bringing the application to the School Department, located at 455 County Street, the youth must also bring their Birth Certificate, along with a picture ID.
IMPORTANT FOR 14 & 15 YEAR OLDS
- All work permit applications must be signed by their physician before submitting to the School Department.
- They must also adhere to the following requirements above (Parental Signature, Birth Certificate, and Picture ID).
*18 year olds DO NOT need a Work Permit for employment.
